If you are a Support Coordinator, Support Worker, Plan Manager, family member, health professional, provider or other person referring someone to Occupational Therapy, please select the option below that best matches how the person will be accessing our services.

NDIS Referral

For participants who are accessing Occupational Therapy through their NDIS plan, please complete our NDIS referral form.

Other Funding / Private Referral

For people who are not accessing services through the NDIS, including private clients and other funding arrangements, please complete our other funding referral form.
